PURPOSE
     The purpose of the Extended Day Enrichment Program is to provide a safe, clean, pleasant, caring, educational, and enriching environment in which each child may develop socially, culturally, emotionally, physically, and mentally.

HOURS
     Our program operates on days school is in session for students and consists of three sessions:
     AM (morning) 7:00-8:15 a.m.
     PM (afternoon) 3:00-6:00 p.m.
                  AM and PM
     The AM session consists of informal, non-structured activities. The PM session provides a more formal, structured program of enrichment activities such as: Arts and crafts, computer, homework assistance, academic enrichment, remediation and movement.
     The Extended Day Enrichment Program will not be in session on Teacher Planning Days, Weather Days and all holidays.  Also, service will not be provided if the school closes due to an emergency.

ELIGIBILITY AND FEES
     Students must be enrolled at Bartram Springs Elementary and only children of working parents may be provided services of the program. Children cannot be absent from school and attend Extended Day. Services are provided on a monthly basis due to eligibility of space in each class/session. THIS PROGRAM DOES NOT PROVIDE WEEKLY OR DROP-IN SERVICES.
 Fees are projected on a weekly rate and are to be paid in advance according to the Payment Schedule received at registration.

ARRIVAL AND DEPARTURE
     A pre-requisite for enrolling your child in our Extended Day Enrichment Program is your guarantee that you will abide by the established times and bringing and picking up your child.
     Teachers do not arrive before 7:00 a.m. and leave the building promptly at 6:00 pm. We regret that this does not meet the needs of every family. Please consider several contact people to pick up your child in case of a traffic jam or other work related situations. We cannot allow for these situations as well as you cannot. Any child not picked up by 6:00 p.m. will be assessed a late fee of $1.00 for every minute PER child. All fees have been approved by the school board. This fee must be paid within 24 hours for that family to continue services. We will be using the school clocks to determine if a child is picked up late, so you may want to set your watches accordingly.
 If your child will be leaving at 3:00 (by walking, car or bus) and will not be in Extended Day for that day, you must send in a note to the classroom teacher. Have your child’s teacher inform us of the change before 3:00.

THREE (3) LATE PICK-UPS WILL RESULT IN YOUR CHILD BEING DISMISSED FROM THE EXTENDED DAY ENRICHMENT PROGRAM.
A.M. ARRIVAL- Parents must bring their children in the  building to the front office and sign them in.
P.M. DEPARTURE- After 3:00 the students are to be signed out and picked up in the front office.
Please look for signs (the first week of school) posted on the front door, to notify you of the assigned areas you can drop off your child and pick up your child.  
                                                
STUDENT DISCIPLINE AND RULES
     All children are expected to follow the rules, as outlined in the Elementary Code of Student Conduct, during the Extended Day Enrichment Program. A Citizenship Warning will be sent home if your child does not follow these rules. The consequences for receiving a Citizenship Warning are as follows:

1st warning:  parent contact, signature
2nd warning: parent contact, signature
3rd warning: parent contact, signature and  week suspension  (PAYMENT NOT REFUNDED)
4th warning: parent contact, signature, suspended from the program (PAYMENT REFUNDED UPON APPROVAL OF SUPERVISORS)
    Input from you concerning problems at home that may affect your child’s behavior during the Extended Day Enrichment Program is welcomed.


HEALTH AND ILLNESS
     Children who are ill should not be brought to school. If you must go to work, then other provisions should be made for the welfare and comfort of your child. If your child becomes ill during the day, you will be asked to come pick up your child.
     Any child that was sent home due to head lice MUST first come to the main office at 8:00 a.m. to be rechecked before he/she may return to Extended Day. NO CHILD WILL BE ALLOWED TO ATTEND EXTENDED DAY UNTIL THEY ARE CLEARED BY THE MAIN OFFICE; AN ADULT MUST ACCOMPANY THE CHILD WHEN BEING RECHECKED.

INSURANCE
     You are encouraged to carry insurance on your child in the event an accident should occur at school. If you do not have a family health insurance plan, you may wish to participate in the Accident Insurance Program Offered by the Duval County School Board.


NUTRITIONAL SNACKS
        In accordance with HRS Guidelines, parents must furnish their children enrolled in the Extended Day Program with an afternoon snack.

WITHDRAWAL
     A WRITTEN NOTICE, SUBMITTED ONE WEEK IN ADVANCE, IS REQUIRED TO WITHDRAW YOUR CHILD FROM THE EXTENDED DAY ENRICHMENT PROGRAM. Your child will be kept on roll and you will be billed unless notification is received. Your child’s enrollment back into the program will not be automatic and will be based upon eligibility available. If you withdraw your child from school you must notify the Extended Day.

ID REQUIREMENTS
     Until staff becomes familiar with you, you will be required to show identification ( drivers license ) when picking up your child. When a staff member is absent and there is a substitute staff member, again you will be required to show identification. Only those listed on the enrollment forms that were completed by you at registration will be allowed to pick up your child. NO PHONE CALLS ARE ACCEPTED FOR PICK-UP CHANGES UNLESS IT IS AN EMERGENCY SITUATION AND WE ARE ABLE TO VERIFY THE CALLER. 


                   CLOTHING AND PERSONAL ITEMS
     Please be sure that your child’s name is on his/her clothing and personal belongings. Emphasize to your child that he/she is responsible for these items, NOT THE TEACHER. Any items not claimed at the end of the week will be stored in the lost and found and parents may claim items there.
     PLEASE DO NOT ALLOW YOUR CHILD TO BRING TOYS OR OTHER ITEMS FROM HOME WITHOUT APPROVAL OF THE TEACHER. WE WILL NOT BE RESPONSIBLE FOR LOST ITEMS BROUGHT FROM HOME AND ACTION MAY WARRANT A PARENT CONFERENCE AND POSSIBLE CITIZENSHIP WARNING.




EXTENDED DAY ENRICHMEN PROGRAM
PAYMENT SCHEDULE
    Your payment schedule is based upon 179 1/2 school days. Your payment is due on the date stated on the payment schedule. Your payment is considered late one day after the due date.  Five (5) school days after the due date a $15.00 late fee WILL be assessed per family.  If payment has not been received by the start of the new payment period, your child will be removed from the program.
     Payments may be made ONLY during hours of operation and ONLY to the Extended Day staff Monday through Friday. NO PAYMENTS WILL BE ACCEPTED IN THE MAIN OFFICE. PAYMENTS WILL NOT BE ACCEPTED BY THE CLASSROOM TEACHER.

     Cash is accepted in EXACT AMOUNT ONLY; overpayment will not be accepted. We do not make change at ANYTIME.
We only accept cash payments or checks.  NO PAYMENTS WILL BE ACCEPTED BY THE U.S. MAIL. WE WILL NOT ACCEPT ANY PAYMENTS BROUGHT TO US BY A STUDENT. PARENTS MUST BRING IN THEIR PAYMENTS TO THE EXTENDED DAY OFFICE OR STAFF PERSON ON DUTY.

Payments are based on 20-day month, 179-1/2 day school year. Holidays and teacher planning days will extend the payment period. *MAY 29, 2017, is a half-day for all students. There will only be morning Extended Day on that day. This is reflected in the April 6th  payment.

“District policy states that students may not remain in the Extended Day Program unless their account balances are paid on a current basis.”
 Make checks payable to Bartram Springs Elementary School- Ext. Day-Please put your child’s name on the bottom of the check.

Important Notice-Please Read Carefully
I have received a copy of the 2019-2020 Extended Day Payment Schedule.  I understand that payments made after 5 school days beyond the due date will be assessed a $15.00 late fee per family.  If my account is not paid at the start of the payment period, my child will not be permitted to come to Extended Day.      Initial_______________

I understand that I must make the payment in person to the Extended Day Office or staff person on duty. NO payments will be accepted by the U.S. MAIL. We will not accept any payments brought to us by a child. No payments will be accepted in the main school office.      Initial_______________

I also understand that checks returned for any reason may be handled by an outside agency. I will be contacted by them(or the Extended Day Director) and make arrangements with them for making reimbursement on a returned check plus all services fees charged.  All future payments will be made in CASH or Money Orders.  Initial__________

If making payment in cash, I understand that I must provide correct change.  The Extended Day office does not keep cash on hand to make change.  Initial ____________

Children who have forgotten homework, planners, etc. are not allowed to return to their classroom  during the Extended Day hours. This rule was made by the school administration and teachers.  I will encourage my child to be organized.  Initial ______________

No child will be called to go home prior to 3:10 PM. NO PARKING is allowed in the bus loading area at anytime (before or after school). Initial _________

Extended Day is not responsible for any lost or stolen items.  Please put your child’s name on jackets, backpacks, lunchboxes, ect.  We encourage parents NOT to allow children to bring Bakagans, Pokeman Cards or other items of value to school.  CD players and MP3 players are not allowed in the Extended Day Program.     Initial ___________

Extended Day does not provide a “drop-in” service.  If an emergency requires non-scheduled care for my child (for example, a child who is signed up for afternoons requiring morning care), I need to make arrangements in advance and the cost of this additional care will be paid at that time.    Initial ________________

I understand that I am responsible for providing my child who attends afternoon Extended Day with a snack every day.     Initial_________________

I understand that Extended Day closes at 6:00 PM.  Parents who are late picking up their child will be charged $1.00 per minute after 6:00 PM.  All late charges must be paid by the following day. If late, you should contact the office by phone to give the office staff and idea of when to expect you.  THREE (3) LATE PICK-UPS WILL RESULT IN YOUR CHILD BEING DISMISSED FROM THE EXTENDED DAY ENRICHMENT PROGRAM. The Extended Day Program does NOT provide Tax Statements of payments made to Extended Day.  My RECEIPTS ARE MY LEGAL PROOF OF PAYMENT.  I will save my receipts. Extended Day staff will not save them for me.
